Client Waiver & Informed Consent

I understand that the services provided are educational and coaching-based in nature and may include fitness training, exercise programming, movement education, performance guidance, and non-medical rehabilitation support.

I understand that these services are not medical care, physical therapy, diagnosis, treatment, or rehabilitation by a licensed healthcare provider unless expressly stated otherwise in writing.

I understand that exercise and physical activity involve inherent risks, including but not limited to muscle soreness, strain, fatigue, loss of balance, aggravation of pre-existing conditions, and other injuries.

I voluntarily choose to participate and knowingly assume all risks associated with participation.

I represent that I am physically able to participate, or that I have consulted an appropriate licensed healthcare provider where necessary.

I agree to stop any exercise or activity that causes unusual pain, worsening symptoms, dizziness, fainting, numbness, chest pain, or other concerning signs, and to seek appropriate medical attention when needed.

I agree to provide truthful and complete information regarding my current health status, exercise history, limitations, pain history, injuries, goals, and relevant background to the best of my knowledge.

I understand that individual outcomes vary and that no specific results are guaranteed.

I understand that online coaching and virtual instruction may have limitations compared with in-person evaluation. I accept responsibility for creating a safe environment for participation, including adequate space, proper footwear, appropriate equipment, and exercise within my limits.

To the fullest extent permitted by law, I release and hold harmless Precision Movement & Performance LLC and its owners, employees, contractors, and representatives from claims, demands, damages, losses, or causes of action arising from or related to my participation, except to the extent caused by gross negligence or wilful misconduct where such limitation is prohibited by law.

This agreement shall be governed by the laws of the State of Wyoming.

By checking the agreement box, signing electronically, or purchasing services, I acknowledge that I have read, understood, and agreed to this waiver and informed consent.
